Quoting and Rates

How do I buy an AultOne policy? Call us at 330-363-6360 or 1-800-344-8858. Or apply online. You may also contact your broker for applications.

How long are these rates valid? Final, medically underwritten, rates for an AultOne policy are valid for 30 days from date of the rate letter.

Do these plans provide dental or vision coverage? No, these plans cover medical claims only. Dental or vision coverage is currently not available.

What ages are eligible for AultOne policies? You must be at least 18 years old, and may not be older than 65. If you’re age 65 or older, we have a Medicare Advantage plan available for you.

Do you offer child-only policies? No. We offer policies for children covered as part of a family, or policies that cover the children only.

Applying for Coverage

What health care services are covered under the AultOne Plans? You may access Schedules of Benefits that are provided under a link on the website.

Do you cover pre-existing conditions? Yes, as long as you provide us a letter of creditable coverage from your prior health plan.

Are maternity benefits covered under the AultOne Plans? Maternity benefits are available as a rider, with an additional cost to the premium. A newborn is automatically covered under the policy for 31 days and can be added, by completing an application, to the policy during this 31-day timeframe.

Who does the maternity rider cover? The maternity rider covers pregnancy-related services for you/your spouse. Maternity riders do not cover the baby once it’s born; the baby must be added to the policy after birth.

Do you require a physical exam for the application? As part of the underwriting process we ask questions about your and your covered dependents’ medical conditions. If we do ask for your medical records, it is your responsibility to facilitate the submission of those medical records to us.

If I want coverage to begin the first of the month, when do I need to submit my application form? We must receive your application form 30 days prior to the effective date you want. For example, if you want coverage to begin on October 1st, we must receive your application by September 1st.

How long will it take to process the application form? Processing your application form may take five to seven business days. Please note that your coverage does not become effective until you return your signed rate sheet to us along with a check for the first month’s premium.

What would cause my application to be delayed? Your application could be delayed for a number of reasons:
  • Application not signed and dated
  • All questions not answered
  • Detail to questions answered "yes" in the medical history section incomplete or not answered
  • Medical records requested and not received yet
  • Other information requested


I am a current AultCare group member. Can I convert my coverage to an AultOne Plan? The AultOne plans are not conversion plans. If you want to leave your group plan, we’d ask you to submit a completed AultOne application. Our underwriters will review that and price out those plans for you. You would then need to terminate coverage on the group plan and buy the AultOne policy.

If you are working with a broker, please submit your application form to your broker. They will then send it to us.

Should I cancel my existing coverage? Do not cancel other coverage currently in force until you and any covered dependents are in receipt of your member ID card(s) providing the effective date of coverage.

Payments and Premiums

How will AultOne bill me? Premiums are automatically deducted from a checking or savings account. We generally withdraw on the 7th, 14th, 21st and 28th of the month. Your chosen effective date will dictate which date your premium is pulled. A schedule of those deductions will be included with your final rate letter.

Do I need to send a premium payment in order to enroll? Yes, you will need to send in a check to cover the first month's premium. This is done after you receive the final rate letter.

Using Your Plan

How do I choose a physician? Please visit our website at https://www.aultcare.com/portal/pls/portal/AULTCARE.MEM_PROVIDER to access an online provider directory.

How can I check on a claim or get information about my coverage? You can check on a claim, request an ID card, and more by logging on to aultcare.com and accessing the Member section of the site.

If I am currently covered under an AultOne plan, who do I contact with questions about my benefits? Members can contact AultCare at 330-363-6360 or 1-800-344-8858.

What Is a Formulary? A formulary is a list of medications put together by the health plan. Using the formulary as a guide, you and your physician can determine which drugs are covered by the health plan and at what level of cost sharing. The purpose is to help you purchase medications based on safety, cost and clinical effectiveness.

Canceling Your Coverage

How and when do I cancel my AultOne coverage? We need a letter, signed by the policyholder to cancel your AultOne coverage. We recommend you cancel at least 10 days before your next month’s renewal date so that we have time to process your cancellation and avoid waiting for a refund. Send a letter requesting cancellation to AultOne at P.O. Box 6910, Canton, Ohio 44706. Please tell us the date that you want your coverage to end.

Can I make changes to my plan once I’m enrolled? You can change your plan only at each renewal (year). You may drop the maternity rider at any time but you can add it only at initial application or at renewal.
